Technology/Chromebook Pick Up & Sign Colorado Residency Affidavit Form

• In the enrollment survey, it states that families must sign this form. This is how we get our funding for our program. Students must live in Colorado to be enrolled in our program. All students will be using the school issued Chromebook for their educational needs.
